TONY ELLIOTT FOUNDATION GOLF TOURNAMENT ENTRY FORM

DATE: Monday, September 18th, 2017

TIME: Noon Registration. 1:00pm Shotgun start

PLACE: Tippecanoe Lake Country Club

ENTRY FEE: $100.00 ($400.00 per Foursome) Includes green fees, cart & catered lunch.

Payment due September 14, 2017. Send check to mailing address below or use Paypal on website: www.tonyelliottfoundation.com.

FORMAT: Tournament will be played in a Scramble format with 4-person teams. If you do not enter a team list, you will be placed with others.

AWARDS: Prizes for 1st, 2nd and 3rd place teams.

CANCELLATION POLICY: If golf tournament is canceled due to weather, we thank you for your donation. If player is a “no-show”, entry fee is non-refundable.
